Some refurbished items may have been defective then fixed, and could still be defective.
In some occasions, manufacturers may actually ship brand new products to buyers of their "refurbished" products.
Yes, some "refurbished" products may actually be identical to brand new products, except at a lower price, if the merchant decides it is cheaper to rebrand new products as refurbished rather than fixing and re-selling defective products.
This marketing tactic can be a way for makers to sell more products to a market segment who waits for discounts, without de-valuing their retail prices.
For example, a professional film maker would be okay with paying full price when buying a new camera lens for their next film project.
A hobbyist would buy the same lens when the price lowers, but manufacturers do not want to de-value their lens to everyone by lowering its price, even for a short time.
Manufacturers may also believe selling new products at a discounted price with less warranty coverage (labeled as refurbished) would be cheaper than fixing, certifying, and re-boxing each product returned to them.
Our experience had been about half of manufacturers ship brand new products when customers buy refurbished, when those items were mass-produced within 6 months and purchases were made directly from the manufacturer.
